APPOINTMENT PROCEDURE

• Members can make an appointment to conduct a check-up at any of the participating hospitals throughout Malaysia.
• Lead-time for the check-up appointment must not be less than 7 days prior to your check-up date.
• Any cancellation or amendment/ re-scheduling of the check-up appointment must not be less than THREE (3) days before the check-up date.
• Confirmation and amendment of check-up appointment is always subject to availability. A confirmation reference code will be communicated.
• A reminder for all appointments will be given (1) day before the check up date.
• Proceed to the Screening Centre at respective Hospitals to register. Please produce your Appointment Confirmation and Preventive Healthcare membership card at the counter.
• The check-up is only available upon fulfillment of required subscription payments and provided that all other annual fee dues are current (Subject to Terms & Conditions of the membership)
